COVID-19 Vaccine Registration

The COVID-19 vaccine is now available in New York to people 12 and older.
Note: People who are 12 to 17 years old are only eligible for the Pfizer vaccine.
You must be eligible to receive vaccine in accordance with the NYSDOH criteria. You must bring proof of age, identification and your insurance card.
Your second dose appointment will automatically be scheduled 21 days after your first dose.
Please reschedule if you are not feeling well on the day of your appointment.
Remember to please wear a mask to your appointment. You will not be vaccinated without one.
Consent for Minors
For the purposes of COVID-19 vaccine, a minor is defined as an individual under the age of 18 years. Minors need parental or guardian consent to receive a COVID-19 vaccine, except in the rare instance where the minor is part of a group to whom the law gives the right to consent to their own care (e.g., emancipated minors, married minors, minors who are parents or pregnant, and minors in the military). A parent or legal guardian must accompany a minor age under the age of 18 years to provide in-person consent for vaccination at each dose. Vaccine Support Staff must document in the the name of the person providing consent for the minor. Verbal consent is allowed.
The delta variant of the virus is continuing to spread rapidly, and vaccination is the most important tool we have to stop this spread. The delta variant is more contagious, more likely to cause severe illness, more likely to reinfect someone who already had COVID-19 and more likely to infect young people.
The following people are now eligible for a booster shot of the Pfizer vaccine, if they received two doses of the Pfizer vaccine at least six months ago:
- People 65 or older
- Individuals 16 years of age and older who have completed a primary series with Pfizer-BioNTech COVID-19 Vaccine at least 6 omnths ago.
- Individuals 18 years of age and older who have completed primary vaccination with a different authorized COVID-19 vaccine. The booster schedule is based on the labeling information of the vaccine used for the primary series.
- People living in a nursing home or other long-term care facility
You will only be able to get a Pfizer booster shot if you received a second Pfizer dose or different FDA authorized COVID-19 vaccine at least six months ago. There are no exceptions to this restriction.
This shot is intended to help many people boost their immunity from the initial vaccination series, which may have decreased over time.
