Community Pharmacies is pleased to team up again with the Yarmouth Community Coronavirus Task force to offer this Free Vaccine Clinic to the Greater Yarmouth Community!

LOCATION: AMVETS 148 North Rd Yarmouth
We will be offering the following vaccines:
- Moderna Adult (Ages 12+)
- Moderna Pediatric (Ages 6mos-5 years)
- Pfizer Adult (Ages 12+)
- Pfizer Pediatric (ages 5-11)
- Janssen (J&J) (Ages 18+)
At your appointment we will need to see your ID and current insurance. If you are not insured we will need to collect your Driver's License number OR SSN. THERE IS NO COST TO YOU FOR THE VACCINE.
Plan on staying for 15 to 30 minutes after your vaccine for observation so we can make sure you do not have a reaction.
Vaccine information sheets as well as Community Pharmacies Vaccine Consent Form are located below. If you are able, please print and fill out a consent form and bring it to your appointment.
If you have a Covid-19 Vaccine Card don't forget to bring it to your appointment so it can be updated with your most current dose.
