Moderna Vaccinations are for all individuals Age 18 & Older.

Thursday, April 22nd will be for Modera vaccines only.

• Complete and Bring the Consent Form, (see related files below) to your appointment. 
• Citizens will only be admitted 15 MINUTES before your appointment time.
• Insurance is NOT required for the FREE vaccine. If you do have insurance, please bring your insurance card.
• You do not need to create an account to sign up for an appointment.
• Those pregnant who want the vaccine must provide their doctors signed consent

How to Schedule your Appointment:

1. Select available Appointment Slot.
2. Click the [Sign Up] button to select your Day and Time. 
3. Click the [Submit and Sign Up] button at the bottom of the webpage.
4. Enter your information, as required, to complete your Sign Up.
