Adolescent Health Program
The Kankakee County Health Department (KCHD) will be hosting an Adolescent Wellness Visit Clinic on Saturday, October 1st, 2022, for Kankakee County adolescents (ages 11-21). This will be a comprehensive appointment to complete your adolescent's school physical, immunizations, point-of-care testing, and/or additional screenings. An Illinois Sport Physical (Medical Eligibility Form) may be completed at this appointment, on request. Your Adolescent's Wellness Visit will be completed through a partnership with Illinois State University- Mennonite College of Nursing and collaborating Family Nurse Practitioners.
Please review the available appointments below and click on the appointment slot to sign up. The "Name" should be the patient, the adolescent, NOT the guardians name (that is only recorded for contact/insurance purposes). The information provided within each sign up is required for us to bill your insurance for the cost of the appointment/immunizations provided to your child on October 1st, 2022. If your adolescent is not covered by insurance, their appointment cost will be $50 (cash or card collected on October 1st, 2022).
You, the Adolescent if over 18, or Parent/Guardian, will be contacted by the Kankakee County Health Department (KCHD) by September 28th, 2022, to confirm your appointment. Please make sure the phone number indicated on this sign up is active and the best number to contact you. [If the KCHD has not been able to confirm your appointment by 4:30pm on September 28th, 2022, you will be removed from the appointment and will not be eligible to receive services on October 1st, 2022]

*If anyone other than the parent or guardian will be accompanying the adolescent (if under the age of 18), please make sure they have a note that states your (the parent/legal guardians) permission for your child to receive immunizations. The note must contain the child’s name and date of birth, the name of the person bringing them, and your name, signature, and date the child will be receiving the immunizations. *
There will be a Mini Health Fair in the Kankakee County Health Department lobby on October 1st, 2022, so you and your family can connect with local providers, learn about Kankakee County services, and receive resources to better your health and wellbeing.

The "name" section should the adoelscents, the guardian information is recorded when indicated for the sign-up. If you are looking to sign up multiple adolescents, EACH adolescent must be signed up for an individual appointment/slot.
