Community of focus: African/African American/Black, Latinx, Asian, Indigenous, Slavic & Eastern European, Pacific Islander, immigrants, refugees and other people of color eligible to receive vaccines in Oregon. This vaccination clinic will be providing the Pfizer COVID-19 vaccine.
Eligibility: All Oregonians ages 12+. For youth ages 12-14, parental consent is required (a) in-person, (b) written consent, or (c) over the phone. Youth ages 15-17 can consent to their own care.
If you are an intended participant for this vaccination event, please schedule yourself for a time below. This is the first step of securing your vaccination appointment. Remember if someone is eligible, they can bring their whole family/household 12 and older to the site as automatically eligible with them. Each person needs their own appointment.
Vaccination for Youth 12+
Youth ages 12+ are now eligible for Pfizer vaccine.
We encourage youth to involve trusted adults in their health care decisions.
15-17 year olds: do not need to be accompanied by an adult and do not require parental consent in Oregon
15 year olds can consent to own medical treatment when providers are physician, physician assistant, naturopath, nurse practitioner, dentist or optometrist
12-14 year olds: must be accompanied by a parent, guardian or someone designated by the parent
If someone other than a parent or guardian accompanies the 12-14 year old, they will need to provide proof of parental/guardian consent. A parent or guardian will also need to be available by phone to answer some medical questions. Proof of consent is either:
A signed consent form (available in English and Spanish) or
A written or typed note that includes the parent/guardian’s name, their relationship to the young adult, the young adult’s date of birth, a statement saying the parent/guardian consents to the young adult being vaccinated, and the parent/guardian’s signature.

OHA Statement on Minor Consent: This states that OHA supports the rights of minors 15 and older to consent to COVID-19 vaccination without parental consent
Please choose your preferred time slot and submit your information. Please keep in mind that these clinics can take an hour or more from start to finish. Please, plan accordingly. To ensure adequate parking, and reduce time waiting in line at the event, please do NOT arrive more than 15 minutes before your scheduled appointment.
