1. READ the below related files. You DO NOT have to print and sign these documents. You just have to read them. Later in this process, you will be asked if you have read and understood these documents.
2. Once you have read the documents below, proceed to select a test date and time and then hit SUBMIT and SIGN UP.
3. You will then be asked to enter basic information about yourself (or your dependent) and provide consent and authorization to proceed.
4. Once you register yourself, confirmation of the date and time you selected will be displayed AND you will have the option to return to the sign up page. Return to the sign up page if you would like to register your spouse or a dependent child. PLEASE ONLY CHOOSE ONE TIME SLOT AT A TIME. ONCE YOU REGISTER YOU CAN RETURN TO THE SIGN UP PAGE AND THEN REGISTER YOUR SPOUSE OR DEPENDENT CHILD.
Please try and arrive 5 to 10 minutes ahead of your scheduled appointment time and be sure to wear your masks!
