Vaccinations at this site will be available by appointment only. Availability is limited and no walk-ins will be accepted. Untill further notice, the form will open every wednesday at 8 am. and close when all appointments are scheduled.

HOW TO SCHEDULE A VACCINATION APPOINTMENT:
1. Determine you are in an eligible category
Please ensure that you meet the vaccine eligibility criteria set by the Wisconsin Department of Health Services: COVID-19 Vaccine Phases and Eligibility (Wisconsin DHS)
16+ only: The Health Department is currently using the Pfizer vaccine, which is only approved for use on people aged 18 or older. If you are under 16 years old, you will not currently be able to sign up for a vaccination appointment at the Health Dpeartment.
2. Schedule an appointment below
Choose an open appointment time below.
Click on the "Sign up" button under the time of your choice.
Click on the "Submit" button at the bottom of the page.
A new page will pop up. Fill in your first name, last name, phone number and email address.
Click on the "Sign up Now" button.
3. Get your vaccine
At your scheduled appointment time, report to the Green Lake County Courthouse, 571 County Rd. A, Green Lake. Enter the building through the Front entrance. Bring a drivers license or form of identification. Please do NOT arrive early.
After you receive your shot you will be required to stay at the center for a minimum of 15 minutes of observation.
4. Schedule your second dose
You will need to be available 21 days later on Thursday, May 6th for your second vaccine. Please wait to schedule your first dose until you can be available for the second dose. If you are unable to make your appointment for your second dose, your second dose is not guaranateed.
It is still possible for you to contract COVID-19 after receiving a vaccination. To ensure the safety of yourself and others, please continue to follow all recommended safety practices such as wearing a mask and maintaining physical distancing.
