**Indoor visitation requires proof of a negative COVID test **
IMPORTANT PRIVACY NOTE:
The information that you enter is visible to everyone who visits that same link. Therefore, to maintain privacy, we ask that you enter your initials in the 'FIRST NAME' field and your loved one's apartment number in the 'LAST NAME' field (your email address will NOT be visible to others).
If you need assistance, a guidebook is attached for quick reference.
Each resident may receive one visit per week (Wednesday - Sunday).
The only exception to this request is, in the event a time slot remains open one hour or less prior to the start time, you may sign up for that slot.
You will receive an email confirmation of your sign up.
Prior to arriving to the community, please review the attached sign with guidelines for the visit.
Upon arriving to the community, please check in with the concierge.
Please bring your N95, KN95 or ASTM level 2 or 3 surgical mask with you and wear it at all times while on the Cedarbrook campus
Upon arriving at the community, please check in 15 minutes before your scheduled time with the concierge
Per state regulations, no more than two visitors per time slot will be allowed - no exceptions
Please note that restrooms will NOT be available for outdoor because we cannot allow you to go beyond the concierge desk without a negative COVID test.
At the conclusion of your visit, we will be cleaning the area and ask that you promptly sign out at the concierge.
