You must have a scheduled appointment, we are not able to take walk-ups.

This is a collaborative vaccine clinic event with Amita St. Mary's Hospital and Kankakee County Health Department.  The Covid-19 vaccine provided at this clinic will be Johnson & Johnson/Janssen One-dose vaccine.

This will be a drive-up clinic (weather permitting).  Please read carefully the instructions for arrival.

Directions for Arrival:

Location: 21 Heritage Drive in Bourbonnais.

Turn west off of Rt 45  (Kennedy Drive) onto Heritage Dr, Bourbonnais.

Please follow the signs into the parking lot. For your safety, there will be reserved parking spots for post vaccination waiting (minimum of 15 minutes).

You will be instructed on where to park and drive.

Please do not get out of your vehicle.

You will exit north to John Casey Rd, once the vaccination process has been completed.

If you are able to print, please have your document completed prior to your vaccination.  The form is attached to this signup (J & J consent).

Please arrive at your scheduled appointment time.

You must be age 18 or older.
