Check In (2) - Greet each parent and child. Have parents make name tag for themselves and child. Retrieve child's chart, look to see if there are any notes, make sure contact info is correct.

Ambassador ( Runner) (3) - Escort child to appropriate appt., invite parent(s) to visit with agencies, take child to first area of treatment. When returning child, let parents know if they failed eye exam and need to go across street for provided full exam.

CVIM Case Manager (2) - Provides parents with information about CVIM services and inquire about family's health insurance. SHare information about CHIP, MA, and Adult Basic. Encourage patients to complete CVIM Intake Form for eligibility.

I am available from 11:30-2:30.
Dental Station #1 - Greet Child, assist them to x-ray station. Give chart to x-ray technician or exam assistant
Eye Screening Station (2) - 1 to track patients, 1 to run patients. Recieve child from the runner or dental area and escort them to the next station.
Check out (2) - Collect patient chart, assure they went to dental and vision appts. If full eye exam is needed ambassador will instruct you. Have parents complete GKAS Survey and collect data for future reference. File chart in box.
Linda K. - Escort patients finished in the dental area to eye area or discharge to family. Provide summary of dental care provided to parent/guardian of each child
Dental sorting - After X-rays are done, check to see if child needs hygiene or restorative, take them to either
Interpreter - Help families and patients communicate with GKAS volunteers. We will need Spanish, and Chinese.
