Eligibility
Hamilton County Health Department is currently providing vaccines to eligible persons who reside in or work in Hamilton County. Proof of eligibility is required.
Currently vaccine supplies are limited; our health department must follow the State’s phased approach to vaccination . Please ensure you or the person you are scheduling meets the current phase(s) restrictions. Anyone not meeting the current phase(s) restrictions will not receive a vaccine.
Hamilton County’s present phases are 1A1, 1A2, 1B or at least 65 years old; only those who are in these phases or age group may register for vaccination.

Making an Appointment
Appointments made for anyone that does not meet the current eligibility restrictions will NOT be vaccinated.
A maximum of two appointments per dose per an email address may be made.
Please complete all information fields when scheduling your appointment. After agreeing to the terms and conditions, entering your information on this site, and selecting an appointment option, you will automatically be scheduled for your selected appointment time. A verification email will be sent to you. Follow the appointment verification instructions and make every effort to attend your appointment. If you cannot make your appointment please use the EDIT MY SIGN UP option to cancel your appointment.
Note: Persons who do not agree to the terms and conditions for using this scheduling option will not be able to make an appointment through this system.
Making a Second Dose Appointment
When scheduling your second dose ensure you schedule an appointment for the same brand you received for the first dose. Refer to your vaccination card for vaccine brand information and second dose date range.
