CAN ONLY RECEIVE ONE FOOD BOX AT CHOPIN HALL PER MONTH

2) Go to bottom of page and click on "Submit and Sign up"

3) Enter your name (Head of Household on CHOPIN Hall Account), address and phone number.  (email is optional)

4) Click on red "Sign up now"
