Currently, City of Springfield Phase 3 Covid19 Road to Recovery order allows thirty-nine people at worship. Please read the screening questions below before proceeding to sign-up. 

SCREENING QUESTIONS 
Do not sign-up to attend if you are experiencing:
 
Fever of 100.4 F or chills; new loss of taste or smell

Cough, congestion, runny nose, difficulty breathing, fatigue, muscle or body aches, headaches

Nausea, vomiting, diarrhea

Traveled out-of-state or out-of-country in the last 14 days

Been in contact in the last 14 days with individual(s) confirmed with Covid19

If you did not answer “YES” to any of the screening questions, please continue to sign-up. 

Instructions to sign-up:
1) “Click” on white SIGN UP square and
2) Continue to SUBMIT AND SIGN UP (bottom of page) and “click” 
3) Enter name(s) and quantity on the SIGN ME UP page (email or Signup Genius account not required) and 
4) “Click” SIGN UP NOW to complete sign up and exit
