ASD Dieruff Location Spring Sports Physicals

Please note, this is for sport athletes on a first come first serve basis.  You MUST bring a completed PIAA CIPPE form (section 1 through 6) to be seen. Sections 7, 8, are only for recertifications.  Sections 6 and 9 are only completed by medical staff.  Physical Forms can be picked up at the school district or downloaded from https://www.piaa.org/resources/forms/default.aspx

All athletes must bring/wear a mask for their exam.

There are no walk ins due to covid-19 protocols.  All social distancing, cleaning, and masking procedures will be observed.

PLEASE NOTE:  Student Athletes with a history of positive covid-19 infection in the preceding 3 month period prior to spring sports physicals must see their PCP or schedule a physical in-office with a sports medicine physician.  They should not go to the mass physical exams but, rather, a private, one-on-one exam where the physician will perform a much more detailed evaluation of the patient. If a student athlete does not have a primary care physician, please see your Athletic Trainer and they will assisst you with getting an appointment.
