COVID VACCINATIONS - BOE
Please review the available slots below and click on the button to sign up for your first dose of the Pfizer vaccination; do not sign up for more than one slot. Please wear a shirt that may be easily rolled up to access the vaccination area on your upper arm. To maintain the safety of everyone, wear a mask during your entire appointment and bring your own ink pen for completing consent forms. To minimize traffic, do not arrive more than 10 minutes prior to your selected appointment. It is also imperative that only the individual receiving the vaccine be present at the appointment.
SCS employees, including charter schools, contracted schools and vendors, are REQUIRED to bring BOTH a state ID and one form of employment verification.
Acceptable Employment Documentation/Verification:
- Employee ID/Badge
- W-2 Form
- Paystub (within the last 2-4 weeks)
- Employment Verification Letter
