Welcome to the HOPEWELL MISSIONARY BAPTIST CHURCH COVID BOOSTER VACCINATION CLINIC On-Line Scheduling tool. It is important to read through these instructions carefully and make sure you follow the steps to insure an efficient vaccination process.
This clinic will provide all the COVID-19 vaccinations including the Pfizer Vaccines 1, 2 and the Booster.
**If you desire a Flu vaccine then this is the place to come and receive the Flu shot on Saturday, October 23, 2021.
The CDC has approved a booster dose for patients who have completed their primary vaccination (2 doses) of PFIZER COVID-19 VACCINE at least 6 months ago and meet any of the following criteria below:
People aged 65 years and older and adults 50–64 years with underlying medical conditions should get a booster shot of Pfizer-BioNTech vaccine.
Residents aged 18 years and older of long-term care settings should get a booster shot of Pfizer-BioNTech vaccine.
People aged 18–49 years with underlying medical conditions may get a booster shot of Pfizer-BioNTech vaccine based on their individual benefits and risks.
People aged 18-64 years at increased risk for COVID-19 exposure and transmission because of occupational or institutional setting may get a booster shot of Pfizer-BioNTech vaccine based on their individual benefits and risks.
Occupations at increased risk for COVID-19 exposure and transmission include front line essential workers and health care workers as previously detailed by the CDC*
- First responders (healthcare workers, firefighters, police, congregate care staff)
- Education staff (teachers, support staff, daycare workers)
- Food and agriculture workers
- Manufacturing workers
- Corrections workers
- U.S. Postal Service workers
- Public transit workers
- Grocery store workers

The gym parking lot is best accessed via Autry Street.
Please do not set an appointment unless you plan on attending. Remember that your clinic time scheduled is a reservation for a vaccination. It is not a guaranteed time when you will receive your immunization. Please be patient and follow the instructions of the clinic staff and we will work diligently to get you through the process as quickly as possible.
- Please bring Photo ID and Vaccination Cards.
- Please bring Insurance Cards.
- Arrive on Time.
