At any age, an illness or injury could leave you unable to make your own health care decisions. Planning for the future is an important step towards making sure you receive the medical care you would want, even when others must make decisions for you.
What is a Health Care Agent? A health care agent is the person you choose to make your medical decisions if, in the future, you are unable to make your own. It's best to have one person serve at a time, with an alternate in case the first person is not available when needed.

What do I need to bring with me? Your Healthcare Agent and Existing Advance Directive/Living Will (if you have one)
