This is an effort to make sure we can contact you in case someone test positive with COVID-19

Please review the following questions

1. Do you or anyone in your household have a new cough or new shortness of breath?
2. In the past two (2) weeks, have you or anyone in your household had a fever? 
3. In the past two (2) weeks, have you been exposed - less than six (6) feet for greater than 15 minutes - to anyone who has tested positive for COVID-19 without wearing the appropriate Personal Protective Equipment (PPE) as defined by the Centers for Disease Control and Prevention?

Only continue if you have reviewed the questions and were able to answer respond "NO" to all three questions. If you have answered "yes" please discontinue.
