Plympton Athletic Youth Sports (PAYS)
This year we will be having a Summer Pick Up Soccer Group for all U10 and U12 kids (Grades 3-6 for the 2019/2020 school year) and all U8 kids (Grades 1 and 2 for the 2019/2020 school year) This will be a fun night of weekly soccer for all boys and girls. Teams will be mixed up as evenly as possible with both boys/girls playing. We will have one volunteer coach at each Summer Pick Up Soccer Day and 1 Referee for each. We are asking every player to bring $2 to play each week so that we can pay the referee for his/her time (or pay $20 up front for all 10 weeks). Payments can be made by check or cash and given at the Pick Up Night to the volunteer on duty. This is 100% for fun and for our players to keep improving on their skills.
Please note that this is NOT a "Drop Off" event. Our Volunteers and Referees are not responsible for watching your children. Our Volunteers and Referees are there to only manage the game. The game will begin promptly at 6:30pm, so please arrive around 6:15pm to warm up and pick teams. A parent or guardian is expected to stay at the field in case of an injury or in case weather forces us to end early.
All players MUST register/sign up here in order to play! This will show us the number of kids attending and allow us to plan accordingly. If numbers are low, we may have to cancel a session, so please be sure to register!
By Registering your child below, you are agreeing to the following:
INJURY AND LIABILITY WAIVER CONSENT
I hereby represent that I am the parent or legal guardian of the child being registered. In return for my child's opportunity to participate in activities organized by the Plympton Athletic Youth Sports (PAYS), I do hereby exempt and release PAYS, its officers, its volunteers, and officials from any and all liability, claims, demands or actions whatsoever arising out of any damage, loss or injury that my child or I might sustain while my child is participating in PAYS organized activities, whether or not such damage, loss or injury results from the negligence of PAYS, its officers, volunteers or officials or from any defective equipment.
PAYS does not provide medical or other insurance for participants, nor are volunteers trained by the league to administer medical care. I recognize that I, not PAYS, am responsible for all of my child's medical insurance and expenses. I further acknowledge that I, as the parent or legal guardian, am solely responsible for consulting with medical professionals to determine if my child's health is adequate to participate in PAYS practices and games.
I understand that PAYS encourages me to be present during practices and games. If I am absent, I hereby give my consent for PAYS volunteers to act for me according to their best judgment in any emergency situation requiring medical attention. I hereby release, discharge, indemnify, and hold harmless PAYS and its volunteers from any and all liability, for injuries or illnesses incurred while participating in PAYS activities. I understand and assume hazards associated with this activity and waive all claims against PAYS.
I understand that if I do not agree to this release, then I will not be able to register my child to participate in the PAYS league. I have read the above and agree to these conditions.
