Once you are signed up, please have your child pick up the paperwork from Classroom D4 beginning June 1st during Lunch or After School. Please have the paperwork completed BEFORE you arrive.

Wear comfortable clothing. If you wear glasses or contacts, please bring them with you.

Payment is due at the time of your appointment by CASH ONLY.
