Offerred by Ascension St. Vincent's Mobile Health free of charge. A notary will be onsite as well. Please schedule an appointment for each student or sibling requiring a sports physical.
Day Of: Arrive 5-10 minutes before your appointment time. Walk-in's will be seen at the provider's discretion. Parents may wait on the bleachers. Have your DoSA sports physical form filled out and brought with you to speed up the process.
