Please review the available slots below and click on the button to sign up. You MUST bring a completed PIAA CIPPE form (section 1 through 7) to be seen. Sections 8,9, and 10 are not required at this time.

All athletes must bring/wear a mask for their exam
PLEASE NOTE: All athletes who have completed their covid isolation/quarantine, are asymptomatic for > 24 hours prior to physicals without medication can be seen at the mass school sports physicals.
