The City of Norman will be hosting a Vaccination Clinc at the 12th Avenue Recreation Center for residents age 65 and older on 1/13/21. The Clinic will be from 8:00am to 2:00pm. To sign up please select a time for your appointment and click submit, then complete the form that follows. 

Residents must bring valid documentation(drivers license, identification card, passport) with their date of birth to validate their age when arriving to get their vaccine.(Proof of US Citizenship is not required) 

You can only sign up for one appoinment at a time.
