Please bring sections 5 (filled out by athlete and parent/guardian) & 6 of the PIAA Physical to be filled out by a physician.
Bring glasses if applicable, or any other supporting documents (i.e. Dr. Notes, Cardiac Clearance, etc)
Social distancing and masks are required.
IMPORTANT: If the athlete has had a positive COVID-19 Test the Physicians will not be able to complete the Sports Physical at the school. The athlete should see their Primary Care Physician (PCP)/Pediatrician to complete the PIAA Physical.
