If you need your first dose of the Moderna covid vaccine and are included in the Group 1 or 2 category: Rural Health Group will be sponsoring a 1st Dose Covid Vaccination event at Franklinton Center at Bricks, 281 Bricks Lane, Whitakers, NC 27891.
The event will begin promptly at 8:30.
**MASKS ARE REQUIRED AND MUST COVER THE NOSE AND MOUTH.**
Group 1 includes:
- Health care workers caring for and working directly with patients with COVID-19, including staff responsible for cleaning and maintenance in those areas
- Health care workers administering the vaccine
- Long-term care staff and residents – people in skilled nursing facilities and in adult, family and group homes.
Group 2 includes:
- Anyone 65 years and older, regardless of health status or living situation
Group 3: Essential Workers
To receive a vaccine:
Sign Up: Please review the available appointment slots below and click "Sign Up" to reserve your spot. The link to this sign up can be clicked more than once to sign up different individuals.
Register: Complete the attached registration forms (found below under "Related Files") prior to arrival. Bring the forms with you to the event. If you require assistance, there will be staff at the event to assist.
Arrive: Please arrive 15 minutes prior to your appointment. Anyone arriving more than 15 minutes before their appointment time will be asked to wait in your vehicle in order to ensure social distancing.
IMPORTANT INFORMATION!!
**If you have received any other vaccines of any kind (flu, shingles, pnemonia, tetanus, etc) within the past 14 days, you must wait at least 14 days to get your Covid vaccine.
**If you have been diagnosed with Covid, you must wait 14 days after being diagnosed as positive AND have no symptoms to receive your vaccine.
**If you have been in the hospital for Covid and received antibody therapy, you must wait 90 days to receive your vaccine.
