April 14, 2021 COVID-19 Mass Vaccination Clinic - J&J

Who is eligible to receive a vaccine?

Individuals 18 and older. 

What vaccine will be administerd?

Johnson & Johnson Vaccine

You are not eligible to receive a Johnson & Johnson COVID-19 vaccine if:

  • You are younger than 18 years old
  • You have symptoms of COVID-19 on the day of the clinic
  • You have been diagnosed with COVID-19 within 10 days of the clinic 
  • You have a history of severe allergic reaction (e.g., anaphylaxis) to a previous dose or component of the COVID-19 vaccine product
  • You have received passive antibody therapy as treatment for COVID-19 within 90 days of the clinic
  • You have received another vaccine within 14 days of the clinic

Vaccine considerations:

There are a few instances in which you should consult with your physician before determining to take the vaccine:

  • If you have a history of severe allergic reaction, i.e.: anaphylaxis.
  • If you are pregnant or breastfeeding, you should first consult with your physician.
  • If you are immunocompromised, you should first consult with your physician.
  • If you recently had an exposure to COVID-19, you should wait until the quarantine period ends.
  • If currently positive for COVID-19, you should defer vaccine until recovered.

What to expect at your appointment:

Your appointment should take approximately 30 minutes. Here’s what to expect during your appointment. 

  • Arrive at JB Wells Expo (2301 CR 197, Gonzales, TX 78629)
  • Please have the forms listed below filled out and bring your photo ID.
  • Follow signs to vaccine registration.
  • Once screened, you will be proved with COVID-19 vaccine education.
  • You will register and then receive the vaccine.
  • After vaccination, you will be monitored for 15 minutes or 30 minutes if you have a history of severe allergic reaction.
  • After 15 minutes you will be released and provided information in case of a reaction.

Please bring the following to your appointment:

  • Photo ID
  • Completed forms attached below

Reminder: Anyone who receives a vaccine still must practice COVID-19 safety protocols, including wearing a mask, social distancing, washing your hands frequently and avoiding large gatherings. Do not be lulled into a false sense of security because you received a vaccine. It takes time for the body to build immunity, and it will take even longer for the global population to achieve herd immunity. Until such time, everyone — including those who were vaccinated and those who have recovered from COVID — must continue practicing safety protocols.

Second dose: Do not sign up here for a second dose. You should arrive for your second dose that is listed on the vaccine card you received at the same appointment time as your first.
