• Bring your child's insurance card (if insured)
• FREE for uninsured children

Ages 3-14

• Must have a prescription from their doctor for vaccines to be given - call your child's physician to inquire about their process for obtaining a prescription to receive vaccines 

Ages 14+

• Does NOT require prescription

• Ages 5 and up - FREE
