Caldwell County is expanding vaccination to those who are considered to be homebound if they have a condition due to an illness or injury that restricts their ability to leave their place of residence except with the aid of supportive devices such as crutches, canes, wheelchairs, and walkers; the use of special transportation; or the assistance of another person; or if leaving home is medically contraindicated advancing the state’s dual priorities of reducing the burden of disease on people at the greatest risk of hospitalization and death and protecting the health care system.
Phase 1B & 1C of vaccination per Texas DSHS will include those 50 and older, 65+ or 16+ with a Chronic medical condition that puts them at high risk for severe illness. These are age groups for which there is strong and consistent evidence of COVID-19’s life-threatening effects. Preventing the disease among people in this age group will dramatically reduce the number of Texans who die from the disease across all races, ethnicities, and occupations.
As Texas progresses into Phase 1C in the coming weeks, Caldwell County will continue to work with vaccine providers and other local partners to ensure that people in Phase 1A, 1B, and 1C have access to the vaccine. Individual providers will have the flexibility to further prioritize vaccination within these groups.
Caldwell County will continue to strive to ensure that vaccine reaches communities with health disparities and people whose work puts them at a greater risk of exposure, per the Texas Vaccine Allocation Guiding Principles. Communication and outreach will encourage vaccine uptake among eligible members of these populations during the Phased rollout.
In preparation for vaccinating people in Caldwell County for COVID-19, we are conducting the following intake application. Your information is confidential and will be used for planning purposes only. This form is not used to schedule an appointment for a vaccination; Caldwell County will contact you to schedule an appointment when we have the vaccine.
For a vaccination, you must first pre-register and complete several questions to determine eligibility. Completing pre-registration does NOT guarantee that you will receive the vaccine or be guaranteed a scheduled appointment. You will receive an email confirming your signup; then, when it's time for you to register for the next available clinic, you will receive an email inviting you to register for the Clinic.
