The last day of the Credit Union COVID Vaccination clinic is scheduled to be Friday July 16 with the clinic ending at 1:00PM. The sign up is full and no longer taking any sign ups. Thank you to all volunteers who have donated their time since we started this clinic in February. We are so grateful for you!

If you are NOT affiliated with UIC or UI Health (i.e. you do not have a UIN and netID), you must fill out a survey at the following website https://uihealth.uic.edu/covid19_vaccine_volunteer_inquiry/ to start the process of becoming a UI Health Team Vaccine Volunteer first before using this sign up list. Only after you have been approved, you will be instructed to use this website.
We need students and faculty who are trained in vaccine administration to assist with vaccination as we seek to expand access to the vaccine for people eligible for the vaccine. You MUST be certifed in BLS CPR. You may also be asked to help out with other IMPORTANT roles in the clinic such as Monitoring, Clinic Flow or Registration. For more details on the specifics on the different roles, please read the Huddle Communication Updates document in the BOX link.
Vaccines will be administered at the Credit Union 1 Pavilion (525 S Racine Ave).
Shifts are from 7:00 - 1:00 and 12:00 - 6:00. Please bring a lunch/snacks as needed. There is a small refrigerator in the break area to store your food if needed. There is no microwave.
When you arrive for your vaccinator shift, please sign in at the Volunteer Table near Gate 3 and check in with the nurses in the Vaccination area. You MUST sign the preceptor attestation form for your shift.

All UIC students and faculty must comply with UIC Campus policy and take a saliva test at minimum every 14 days in order to volunteer at the Credit Union Arena. Click here for test locations and appointments https://go.uic.edu/cvtestbook
Please consider carefully whether this time commitment will fit your schedule or other life commitments. You must have the willingness and ability to make a commitment to your volunteer shift that you signed up for. We are depending on you and so when you sign up and commit to a shift, we expect that you will show up (unless you are not well- if you are not feeling well please stay home!). It really hurts us when our volunteers no show, so please be courteous and respectful.
