Pfizer Booster shots are available for those who are 12** years or older and are 5 months or greater from their 2nd dose or 2 months or greater from receiving Johnson & Johnson.
Please bring consent form and vaccination card to your appointment. Please only arrive 5 minutes prior to your appointment. Masks are required.
**12-17 y/o's MUST have written parental consent on consent form and an ADULT accompaning them.
